ころなういるす わくちん ちゅうしゃの うけかた／How to get the COVID-19 Vaccination in Hitachi City 

ひたちし から あなたの いえに わくちん ちゅうしゃの てがみを おくり ました

●てがみ：わくちん ちゅうしゃを する ときに つかいます。ぜったいに すてないで ください！
　Hitachi City sent a letter regarding the vaccination to residents aged 12 to 64.
　This contains very important documents necessary to receive the vaccine. DO NOT throw it away!

●てがみの なかに はいって いる もの
　You will find these in the envelope. Please find the following 3 documents enclosed.

　(1)　くーぽん けん（せっしゅ けん）
　　　 Vaccination tickets (Coupons for two doses of the vaccine)

　(2)　せつめいの かみ（1まい）
　　　 Instructions about the vaccine and making an appointment

　(3)　ちらし（どちらか 1まい）
　　　 Leaflet for information on the vaccination procedure by Hitachi City (either one of the two)

※12さい～15さいの ひと には 「よしん ひょう」（2まい） も おくり ました。
　「よしん ひょう」＝あなたの けんこう についての しつもんの かみ
　16さい いじょうの ひとは かいじょうで「 よしん ひょう」を かき ます。
　Hitachi City sent "Yoshin-hyo" preliminary medical questionnaire form (2 copies) to residents aged 12 to 15.　
　Residents aged 16 and over write preliminary medical questionnaire form at vaccination site.

たいせつ／Important Notice

●わくちん ちゅうしゃは むりょう（0えん） です。
　The vaccination is free of charge.

●にほんに すんでいる がいこくじんは だれでも できます。
　The vaccination is available for all foreigners who live in Japan.

●わくちん ちゅうしゃは おなじ わくちんを 2かい ちゅうしゃ します。
　In order to get the full effort of the vaccine, it is necessary to receive two doses of the same type of vaccine at a set interval.

●1かいめの わくちん ちゅうしゃ のときに、つぎの わくちん ちゅうしゃの ひを きめ ます。
　On the day of your first vaccination, you will receive the appointment for the second dose that is the same place and same time as the first one.

●8がつ 31にち （かようび） 8：30から わくちん ちゅうしゃの よやくが できます。
　Appointment will start on Tuesday, September 3 AM8:30.

　※8がつ 25にち（すいようび）に よやくの おしらせの はがきを おくり ました。
　　Hitachi City sent a postcard regarding the vaccination appointments on August 25.

　　にんしん している ひとは、じぶんの びょういんで そうだん して ください。
　　If you are pregnant, possibly pregnant, please consult your family doctor before receiving the vaccination.

●よやくは すこし むずかしい です。にほんごが わかる ひとに きいて ください。

　To receive the vaccination more easily, please wear short sleeves, if possible.
